J’ai le cœur très lourd en écrivant cette feuille d’information. Depuis 1990, je travaille à la Clinique Hillside et, en collaboration avec mes partenaires et le personnel administratif, j’ai été très chanceux d’y bénéficier d’un environnement merveilleux. Mais l’immeuble a été vendu et ne pourra plus héberger une clinique médicale. Soyez assurés que, malgré les changements prévus, je ferai tout mon possible pour continuer à vous offrir la même qualité de soins que celle fournie depuis les 27 dernières années.

Le présent document décrit la nouvelle façon de fonctionner. Certaines choses seront différentes, mais au bout du compte, vous allez entrer dans mon nouveau bureau, vous asseoir face à moi dans une nouvelle chaise et je vous dirai quelque chose comme « Bonjour, en quoi est-ce que je peux vous aider aujourd’hui ? »

Grandes lignes de mon nouveau fonctionnement

Actuellement, à la Clinique Hillside, je reçois des patients 5 matins par semaine. À partir de la semaine du 9 avril 2018, je vais recevoir des patients 4 matins par semaine et je serai à McGill la 5e journée.

Je travaillerai sur 2 sites différents : la Clinique Roper et le Centre de soins primaires Reine Elizabeth (3e étage).

Points importants à propos de ma Clinique sans rendez-vous au Reine Elizabeth :
• La Clinique sera ouverte de 8 à 9 h
• La Clinique est située au 3e étage
• Ne vous présentez pas au 1er étage de la Clinique de soins urgents (Urgent Care Clinic) du Reine Elizabeth
La Clinique Roper

Cette Clinique possède une excellente équipe. J’ai déjà travaillé avec la plupart des médecins et, pour moi, la collaboration avec ce groupe de médecins sera la partie la plus facile du changement. Toutes les personnes qui s’occupent des patients (réceptionnistes/secrétaires, personnel administratif, infirmières) sont chaleureuses, compétentes et professionnelles. Tout le groupe est motivé pour faciliter le changement et le simplifier le plus possible pour tous.

Les caractéristiques physiques de l’immeuble seront les principales différences. À l’entrée, il y a environ 10 marches sans rampe d’accès et la salle de toilettes n’est pas accessible aux personnes handicapées. Donc, je ne pourrai pas recevoir ici mes patients qui présentent des problèmes de mobilité. Cependant, je travaillerai les vendredis au Complexe de santé Reine Elizabeth pour y recevoir les patients qui auraient des difficultés à naviguer à travers la réalité physique de la Clinique Roper. Le stationnement est un problème partout à Westmount, mais les places se libèrent rapidement puisque la clinique est très occupée, donc arrivez tôt et prenez patience !

Le Centre de soins primaires du Complexe de santé Reine Elizabeth

Je me joindrai à ce groupe les vendredis matins et je travaillerai au 3e étage. Tout comme à la Clinique Roper, l’équipe du Centre est formidable. Je vais recevoir moins de patients sur rendez-vous, mais je recevrai les patients de la clinique sans rendez-vous de 8 à 10 h. C’est le seul endroit où je peux recevoir mes patients présentant des problèmes de mobilité. Mon but n’est pas de diminuer le nombre de mes rendez-vous au Reine Elizabeth. Vous pouvez décider de me consulter ici parce que c’est plus près du métro ou plus près de chez vous ou bien que vous allez manger au PFK tout aussi bien que parce que vous avez des problèmes de mobilité. Peu importe la raison, si vous préférez me consulter au Reine Elizabeth, n’hésitez pas à y prendre un rendez-vous.

Cliniques sans rendez-vous

Comme indiqué ci-dessus, je recevrai des patients en consultation sans rendez-vous de la même manière qu’à la Clinique Hillside. À la Clinique Roper les lundis, mercredis et jeudis, de 8 à 9 h et au 3e étage du Complexe Reine Elizabeth les vendredis de 8 à 10 h.

Ma disponibilité sera mise à jour toutes les semaines sur le site web de la Clinique Hillside.

Les mardis, les personnes qui ont besoin de soins urgents ont 3 options. À la Clinique Roper ainsi qu’au 3e étage du Complexe Reine Elizabeth, les médecins et infirmières sont disponibles de 8 h 30 à 9 h et il y a toujours la Clinique de soins urgents (Urgent Care Clinic) du Complexe Reine Elizabeth qui est ouverte 8 h à 20 h.

Depuis quelques années, je reçois environ 10 patients par jour en consultation sans rendez-vous et ce nombre peut grimper à plus de 15. Les 2 cliniques où je travaillerai désormais ne sont pas habituées à ce nombre de patients. Je vous demande d’être patient et de nous donner le temps de nous adapter à ces nouveaux volumes de patients.

Nous sommes toujours en négociation. J’espère reproduire le même système que celui qui était disponible à la Clinique Hillside. Il reste encore plusieurs problèmes à résoudre.

Résultats et prescriptions (TRÈS IMPORTANT)

Je suis très préoccupé par la perte potentielle de résultats d’analyse qui pourrait survenir pendant le déménagement. Si en 2018, vous avez subi une analyse de sang, une radiographie, une culture de selles, un test PAP, etc., vous allez recevoir une réponse de ma part. Si vous n’en recevez pas, présumez que je n’ai pas vu les résultats. Je vais faire de mon mieux pour faire tous les suivis, mais le meilleur système consiste à partager cette responsabilité.
Si vous allez subir un test que j’ai prescrit sur une requête de la Clinique Hillside dans les derniers mois, prière d’informer la secrétaire que mon adresse a changé et aussi modifier l’adresse sur la requête. Ceci vaut également pour les consultations chez un spécialiste : prière de les informer que mes coordonnées ont changé (adresse, téléphone et télécopieur).
Vous devriez aussi informer votre pharmacien.

Assiduité (fidélité des patients)

L’assiduité est un mot utilisé par le gouvernement pour mesurer la fidélité des patients à leur médecin et à son groupe de médecine familiale. Un système bureaucratique complexe a été mis en place pour encourager les médecins à offrir plus de disponibilités à leurs patients. Cependant, ce système pénalise le médecin lorsqu’un de ses patients consulte un autre médecin qui ne fait pas partie de son groupe. En d’autres termes, si un de mes patients qui habite LaSalle consulte avec son enfant une clinique sans rendez-vous locale pour un problème mineur, ceci joue contre moi. Si 20 % des consultations de mes patients sont réalisées par un médecin autre que moi ou un médecin affilié avec mon groupe de médecine familiale du Reine Elizabeth, alors je serai pénalisé.

C’est pourquoi il est important, pour les consultations en soins urgents, d’utiliser soit la Clinique de soins urgents (Urgent Care Clinic) du Reine Elizabeth qui est ouverte 7 jours par semaine, soit l’une de mes deux cliniques pendant les jours de semaine.

Ce qui est le plus difficile à vivre dans ce changement, c’est de perdre la famille que nous avions créée.
Kathy a travaillé avec nous pendant 18 ans. Elle a fait preuve d’un formidable esprit d’équipe et a été une chef de bureau loyale. Ensemble, nous avons vu grandir nos enfants et vieillir nos parents. Sans ses compétences et son assurance, il nous aurait été impossible d’offrir le même niveau de qualité des soins. Malheureusement, les cliniques où je déménage n’ont pas besoin de nouvelle secrétaire.
Marlene travaille avec nous depuis 5 ans. Son incroyable énergie positive et son sourire toujours présent ont été contagieux et grandement appréciés par les patients et l’équipe médicale.
Docteure Josephson déménagera dans un bureau sur la rue Westminster dans l’ouest de Montréal et mettra fin à sa pratique médicale plus tard en 2018.
Docteur Wiegens déménage dans un bureau sur la rue Sherbrooke.

TRANSITION UPDATE FOR DR. JOSEPHSON         MARCH 2018

As of April 10, 2018 I will be located at a new address for the following 6 months.  The set-up is not the same as that we have been used to at Hillside.  Please note the following;

  • The secretaries at Hillside are doing any bookings for the new location until March 29.
  • The week of April 2 I will be doing the move and not available for appointments until the following week.
  • There is no walk-in available at the new location.  Seeing Dr. Josephson is by appointment only. I will try to keep a few time slots available to give for urgent visits.  You can call the new office location after April 2  and ask if there is an opening available.  I will do my best to accommodate last minute visits.  The days I am able to book appointments are Tuesday, Wednesday and Thursday.  
  • Please DO NOT SHOW UP TO BE SEEN WITHOUT AN APPOINTMENT.
  • THERE ARE NO FACILITIES AT THE NEW LOCATION TO SEND BLOOD TESTS, ETC.  Any tests to be done will require a requisition from me so you can go to your local hospital or CLSC for the tests.   I will only be able to do and send PAP tests for the first few months.
  • If you have a question or are unsure of what to do,  you can certainly leave a message and I or one of the secretaries will return your call.
  • If you need a regular medication renewed (that I have prescribed),  please ask the pharmacy to fax the request to 514-369-1234 to my attention.
  • Referrals to specialists should be with your new MD unless I have already assessed you for this issue.  Remember that new referrals will take time to get appointments and I will not be available for follow up so this is best handled with a new MD.
  • Yearly renewals of referrals for insurance coverage for massage therapy, etc should be done with your new M.D., not me.
  • ORIGINAL CHARTS will be kept with me after closure.  If there is a need for any information from your chart you may send me a request at drdjosephson@gmail.com
  • I have rented this temporary location in order to provide a smooth transition for my patients.  For any questions, issues we can work together to find a solution.  Please respect and have patience that the new secretaries will not know you or me well or how we have practiced previously.
  • For your new MD, remember that most blood tests and radiology results are available to MD’s through the DSQ site  (dossier sante du quebec).  For many of you this is all the information they need.

Dear Patients;

It is with much hesitation that I am writing this letter concerning my decision to gradually close my office after 35 years in medical practice. Several events have led to this decision. Our clinic building has been sold and we have been informed that we must vacate the building by April 1, 2018. Too short a time but we are doing our best. As a group the dynamics have changed with the retirement of one physician and the decision of another to change his practice set-up.

None of this negates the absolutely fabulous medical practice I have had at Hillside. Family medicine is a truly unique and privileged profession. At Hillside we have had a wonderful opportunity to deliver medicine in a caring manner and often with a healthy dose of laughter. So many laughs, cries, upsets, challenges. To have a patient whom I took care of as a young child later bring me her baby to care for is a unique experience.
To every one of you I say thank you for enriching my life and reaffirming what I have always believed, that practicing medicine is a golden opportunity to connect with people in a positive way, (and perhaps saving a few lives as well). And rest assured, there are many wonderful doctors out there working hard to provide the best of care to their patients.

TRANSITION INFORMATION

Dear Patients;

So, in dealing with this change I will do my best to accommodate my patients at this time of transition. A few points;
• Those of you already booked for appointments will have that appointment honored. If it is booked after April 1st , when we have to leave, I may be able to reschedule you earlier; if not then at my new temporary location. Please wait for the secretaries to be in touch about appointment changes.
• If you need to make a follow-up appointment with me for 2018, I will try to see as many of you as possible at Hillside until April 1st or if needed appointments will be booked at my new location.
• Under medicare we work on a registered patient basis. When you find a new doctor you will be required to sign with that doctor which automatically deregisters you from me. Thus your care is in the hands of the new physician and I will no longer be able to follow you.
• If you require any information in your chart, a short summary will be provided with a copy of your latest test results and any other significant information. There will be a nominal fee, usually of $20, to prepare a copy. Chart information can be scanned, faxed, or picked up as a paper copy (preferred method). Details will be on the website. For many of you your vaccination record and a list of any medications (which can also be produced by the pharmacist) are what’s most important. Remember that many of your blood test results and radiology reports are now available to another physician through the DSQ (dossier santé de Québec).

• We are required to keep an original medical file for 5-7 years. In future, information on the location of the medical files will be either on the website (hillsidefamilymed.ca) or registered with the College des Médecins du Québec
• I will provide an email address to contact me after the move. I ask that it be used judiciously, primarily to inform me if you need a copy of your chart.
• As I am still in the process of finding temporary space, please check our website regularly for important updates. The website will be kept going even after April 1st to keep you informed.
• I am working on a list of clinics/doctors who are taking new patients. This will be updated on the website. Many of you will also be able to find a family physician through family and friends.

I will be providing all this information and more on the website. As our secretaries will be very busy, please use the website as the initial point of information.

Lastly, I will be happy to hear from any of you re comments, questions or just a quick hello.

The Closure of the Hillside Family Medicine Practice and the Implications for the People Followed by Dr. Barry Slapcoff
(March 19th 2018)

It is truly with a heavy heart that I am writing this information sheet. I have been working at the Hillside location since 1990, and with my partners and administrative staff, have been fortunate to work in a wonderful environment. However, the building has been sold and will no longer be operated as a medical clinic. Please rest assured that despite the upcoming changes I will do my best to continue to deliver the same level of care that I have been able to provide for the past 27 years.

This document will include a description of how I plan on functioning going forward. There are things that will be different, but in the end you will come into my new office and you will sit in a new chair across from me, and I’ll say something like “Good morning, how can I help you today?”

Broad Strokes-Where I’ll be working

Presently I book patients 5 mornings a week at Hillside. Starting the week of April 9th , 2018 I will be seeing patients 4 mornings a week and working at McGill on the 5th day.

I will be working at 2 different sites, The Roper Clinic and the Queen Elizabeth Primary Care Clinic (3rd Floor).

Please note some important points regarding my QEH Walk-in Clinic:
• The Clinic will run from 8:00 until 10:00 a.m.
• The Clinic is on the 3rd floor
• Do not go to the 1st floor QEHC Urgent Care Clinic

The Roper Clinic

I will be joining an excellent team at this clinic. I have worked with most of the doctors previously and for me, collaborating with this group of physicians will be the easiest part of my transition. Likewise the receptionists/secretaries, administrative staff, and nurses who deal with the patients, are warm, competent and professional. The entire group is motivated to help make the changes for all of us as seamless as possible.

The biggest difference will be the physical features of this building. There are about 10 steps with no ramp in front of the building and the bathroom is not handicapped accessible, therefore I will be unable to see my patients with mobility issues here. However I will be working at he Queen Elizabeth Health Complex on Fridays to accommodate those people who have challenges navigating the physical realities of the Roper Clinic. Parking is a concern for everybody in Westmount but because this is a busy medical clinic there is a fairly high turnover, so arrive early and be patient.

The Queen Elizabeth Health Complex-Primary Care Group

I will be joining this group on Friday mornings and working on the 3rd floor. Like at the Roper Clinic there is a great team working here. I will be seeing fewer patients for appointments, but I will run the walk in clinic from 8:00 to 10:00. This is the only place that patients with accessibility issues can see me. However, my plan is to not restrict who books appointments at the QEHC. People may choose to book an appointment with me here because it is closer to the Metro, or because they live within walking distance or because they are going to KFC for lunch around the corner as well as because of mobility issues. Whatever the reason, if you would prefer to see me at the QEHC (3rd floor Primary Care Clinic) don’t hesitate to book an appointment.

Walk in Clinics

As was mentioned above I will be available for Walk in visits without an appointment in much the same way that I was at Hillside. On Monday, Wednesday and Thursday from 8:00-9:00 at the Roper Clinic and on Fridays from 8:00-10:00, on the 3rd at the QEHC.

My availability will be updated weekly on the Hillside website.

On Tuesdays people needing urgent care services have 3 choices. At both the Roper Clinic and the QEHC 3rd floor clinic doctors and nurses will be available from 8:30 to 9:00 and there is always the QEHC Urgent Care Clinic that is open from 8:00 a.m. to 8:00 p.m.

Over the last few years I have averaged about 10 walk in visits per day and have not infrequently gone over 15 visits. The 2 clinics where I will be working are not used to seeing this volume of patients. Please be patient as we figure out ways to handle what we expect to be higher volumes.

Blood Tests

This is still being negotiated. I am hopeful to recreate the same system that I had at Hillside There are multiple issues that remain to be worked out.

Results and Prescriptions (VERY IMPORTANT)

I am very concerned that results of tests will be lost during the move. If in 2018, you have had a blood test, an x-ray, a stool test, a culture, a PAP test etc. you will hear from me. If you do not hear from me please assume that I have not seen the result. I will do my best to keep track of everything but the best system involves having this as a shared responsibility.
If you will be having a test that I ordered in the last few months with the Hillside requisition please inform the secretary that my address has changed and change the address on the requisition. Assiduity (Patient Loyalty)

This is a word used by the government to measure patient loyalty to their doctor and the doctor’s Family Medicine Group. A complex bureaucratic system has been developed that has the goal of encouraging doctors to be more available to their patients. However the way this is done is by penalizing doctors when a patient of a doctor sees other doctors who are not affiliated to his/her group. In other words if one of my patients who lives in LaSalle takes their child to a local walk in clinic for a minor problem that would count against me. If 20% of my patients’ total visits are to doctors who are not affiliated with the QEH Family Medicine Group, or me, then I will be penalized.

Therefore, it is important, that if necessary for urgent matters, please use the QEHC Urgent Care Clinic 7 days a week, or either of my other two clinics during the week.

The hardest part of leaving Hillside is that the family we created will disband.
Kathy has worked with us for 18 years. She has been a great team player and loyal office manager. We have watched each others children grow up and parents grow old. We could not have delivered the high quality of care that we did without her poise and excellence. Unfortunately the places to which I am moving do not have place for new secretaries
Marlene has only been with us for 5 years. Her ever-present smile and incredible positive energy have been infectious and greatly appreciated by the patients and the medical team.
Dr. Josephson will be moving to an office on Westminster in Montreal West and then closing her medical practice later in 2018.

Transferring your file:

We will be happy to provide you with copies of relevant material from your file, at a fee of $20.00 to take with you to your new doctor.  You should be aware that most recent (past couple of years) laboratory and radiology results as well as your medication record are already available via the internet to authorized individuals (e.g., your new doctor).  The original file will remain with Dr. Lysy and will be available if the need should arise.

Nous sommes ouverts du lundi au vendredi de 8 h à 9 h pour les soins d’urgence. Pour connaître l’horaire des médecins disponibles pour les soins d’urgence sans rendez-vous, veuillez consulter la page de rendez-vous et des heures d’ouverture.
Les tests peuvent être effectués habituellement par votre propre médecin lorsque celui-ci est disponible lors des cliniques d’urgence sans rendez-vous ou lors d’un rendez-vous. Veuillez vous référer à la page Internet Emplacement et heures d’ouverture. Des frais de 20 $ sont habituellement requis pour le transport des échantillons. Sinon, tous les tests sont disponibles gratuitement à votre hôpital local ou dans votre CLSC.

Urgent Care Walk-in (Dr. Slapcoff only)

The urgent care walk-in is only available to registered patients of the clinic. Patients may come between 8AM and 9AM without an appointment. On statutory holidays and weekends, urgent care is available at the Queen Elizabeth Health Complex. Please see below for the urgent care walk-in schedule. Dr. Slapcoff is usually available on Monday, Wednesday and Thursday and the Roper Clinic, and on Fridays at the QEHC. Doctors and Nurse practitioners MAY be available to see you in his absence. If visiting the urgent care clinic on a day when Doctor Slapcoff is unavailable, please use the Roper Clinic Monday to Thursday and the 3rd floor QEHC clinic on Fridays. If the medical team is unable to see you they will direct you to the 1st floor QEHC clinic

Parking is limited.

Please ensure that you have reviewed the following information.

  • Bring a valid up-to-date medicare card
    • Note: If your medicare card is expired or lost, please contact your local CLSC for replacement: santemontreal.qc.ca. If your medicare card is expired, you will be required to pay for your visit.
  • Bring immunization records to checkups ***
  • An up-to-date list of all current medications
  • For blood tests: Fast for 12 hours prior to the blood test. You may drink water.

Our Health Services

For specific information on blood tests, vaccinations, prescription renewals, and other services, see below.

We are open daily Monday to Friday between 8-9AM for urgent care. For the schedule of physicians available at urgent care walk-in, and holiday hours, please refer to the Appointments & Hours page.
Tests may be done usually by your own MD when available during urgent care walk-in or during an appointment. Please refer to the Location & Hours page. There is usually a $20 charge for transport of specimens. Alternatively, all tests are available free of charge at your local hospital or CLSC.
Recommendations for screening should be discussed with your MD. Fit Test Directions
You can renew your medications during a visit with your MD, or if necessary, ask your pharmacist to fax us at (514) 931-8436 with the prescription renewal request.
  • Driver’s license form
    • Please note that they have a due date.
    • A physical exam must have been done within one year in order for the form to be completed without a visit. Otherwise, you must make an appointment.
    • If you cannot get an appointment before the due date, call the SAAQ and request an extension. There is a charge of $30 for completion of the form. – SAAQ Website
  • There may be a charge for certain items not covered by RAMQ. These are charged according to provincial guidelines.
  • Payment is by cash or cheque payable to the individual physician.
  • New referrals will usually require you to be seen by your MD.
  • Heather Nathens, a professional dietitian and certified diabetes educator is available for consultation for a fee. For an appointment, please call (514) 931-2162.

Hillside Family Medicine (Permanently closed as of April 6th, 2018)

 

 

To the Patients of Hillside Family Practice:

December 2017

This letter is to advise you of the unfortunate situation that has developed due to the recent sale of the building that houses our medical practice.   We have been informed that we must vacate the premises by March 31, 2018. Therefore there will be important changes within the family practice group.

 

Dr. Josephson will be relocating temporarily to a new office after March 31st and then slowly closing her practice by September 2018. The temporary location will soon be known and the information will be communicated via email, our website and our secretaries. When available there will be further information re future visits, charts, etc. She is trying hard to find a replacement doctor for her practice in the area. Please refer to the separate letter concerning her practice.

 

Dr. Slapcoff and Dr. Weigens will be moving their practices to new locations which they too will soon be able to announce. They have full practices and are not able to take on any of Dr. Josephson’s patients.

 

For now until approximately March 31, 2018 all 3 practices will continue at the current location
